Dengue Is Expanding: Why Mosquito-Borne Viruses Are Hard to Control
Dengue is caused by four related dengue viruses and is spread mainly by Aedes mosquitoes. These mosquitoes often live near people and can breed in small amounts of standing water. Dengue is common in tropical and subtropical regions, but travel, urbanization, climate patterns, and mosquito expansion are increasing concern in more regions.
Dengue outbreaks happen when three conditions come together: mosquitoes capable of transmitting the virus, people who are susceptible, and active virus circulation. Because there are four dengue serotypes, a person can be infected more than once. A second infection with a different serotype may increase the risk of severe dengue in some people, which makes vaccine development more complex than for many other viruses.
Symptoms may include high fever, severe headache, pain behind the eyes, joint and muscle pain, nausea, rash, and fatigue. Most people recover, but warning signs such as severe abdominal pain, persistent vomiting, bleeding, difficulty breathing, or extreme weakness require urgent medical care. WHO notes that there is no specific treatment for dengue or severe dengue, but early detection and proper medical care can greatly reduce fatality risk.
Prevention still depends heavily on mosquito control: removing standing water, using window screens, wearing protective clothing, and applying insect repellent. Communities also need surveillance to identify outbreaks early.
Vaccines are improving, but dengue vaccination is not simple. In the United States, Dengvaxia has had restricted use in children with laboratory-confirmed previous dengue infection living in endemic U.S. territories. Globally, WHO recommends Qdenga in certain high-transmission settings. A major recent advance is Brazil’s Butantan-DV, a single-dose tetravalent vaccine approved in Brazil for people aged 12–59, with five-year Phase 3 data showing durable protection against symptomatic and severe dengue outcomes.
Take-home message: Dengue control cannot rely on one tool. The best strategy combines mosquito control, personal protection, rapid clinical care, and carefully implemented vaccination programs.
