Why Is Measles Coming Back?

Measles was declared eliminated in the United States in 2000, but elimination does not mean the virus disappeared from the world. When measles is imported through travel and enters communities with low vaccination coverage, outbreaks can occur quickly. In 2026, the U.S. has already reported more than 1,800 confirmed cases and multiple outbreaks, showing how fast measles can return when immunity gaps grow.


Measles spreads through the air when an infected person breathes, coughs, or sneezes. It is far more contagious than many other respiratory viruses. A person can spread measles before they realize they are seriously ill, and the virus can remain infectious in the air or on surfaces for a period of time after an infected person leaves the room. That is why outbreaks often occur in schools, childcare centers, churches, clinics, airports, and other shared spaces.


The main reason measles is returning is not vaccine failure. It is usually under-vaccination. The MMR vaccine works very well: CDC estimates that one dose is about 93% effective and two doses are about 97% effective against measles. But because measles spreads so easily, communities need very high vaccination coverage to protect infants, immunocompromised people, and others who cannot safely receive live vaccines.


Symptoms often begin with fever, cough, runny nose, and red eyes, followed by a rash. There is no specific antiviral treatment for routine measles infection. Care is mainly supportive, and severe complications can include pneumonia, brain inflammation, hospitalization, and death.


There is also promising research on measles antibodies. Scientists recently described human monoclonal antibodies that target the measles virus H and F surface proteins, which the virus uses to enter cells. These antibodies may eventually help protect people after exposure, especially those who cannot be vaccinated, but they are still not a substitute for MMR vaccination.


Take-home message: Measles is a warning sign. When vaccination coverage falls, old diseases can return. The safest and most effective protection remains timely MMR vaccination.
